Health and contact information.
To care for you, we collect and keep the health information we need, including your medical and medication history, allergies, treatment records, insurance and billing details, and contact information. We protect it as required by federal and state law, including HIPAA.
Forms, and how the site is used.
- Forms: when you register, ask us to call you or check your coverage, the details you enter are sent over an encrypted connection to our secure patient-intake system, and used only to contact you, prepare for your evaluation and check your insurance.
- Analytics and advertising: like most websites, we use tools such as Google Analytics and advertising measurement to understand which pages are useful and how people find us. These collect technical information such as your browser, device and pages visited, not your medical information.
- We never sell your personal or health information.
Treatment, payment and running the practice.
- Treatment: to provide and coordinate your care, including with your other clinicians, pharmacies and laboratories, and with the Spravato safety program (REMS) as federal rules require.
- Payment: to bill your insurer, request prior authorizations, appeal denials and enroll you in manufacturer support programs you’ve agreed to.
- Operations: for quality improvement and staff training.

- With family or friends, when you agree.
- For public health, or when the law or a court requires it.
- To prevent a serious threat to someone’s health or safety.
What you can ask us for.
- Access: copies of your records.
- Amendment: corrections to your information.
- Restriction: limits on how we use or share it.
- Confidential communication: that we contact you a particular way.
- An accounting of certain disclosures we’ve made.
- Notification if your information is ever breached.
Safeguards, and who to ask.
We use physical, electronic and procedural safeguards: encrypted records, access on a need-to-know basis, regular staff privacy training, and secure offices.
