How long Spravato takes to work

How long does Spravato take to work? Often days, not months.

Spravato can start working within days. In the main approval trial, people on Spravato were already doing better than the comparison group the day after their first dose, and the difference held through the first month. Compare that with the 6 to 8 weeks a daily antidepressant usually asks you to wait.

When does Spravato start working?

Spravato often starts working within the first days to weeks of treatment. In TRANSFORM-2, the trial that led to its approval, people on Spravato plus an antidepressant improved more than people on an antidepressant alone from day 2, and the gap was clear at week 4 (Popova et al., American Journal of Psychiatry, 2019).

The first daysSome people feel lighter or clearer within a day or two of the first doses.Treating twice a week and watching how you respond.
Weeks 1 to 4Most people who respond see the change build across the first month.Checking in at every visit on mood, sleep and energy.
Week 4A natural point to judge how well it's working.Deciding with you whether to continue, adjust or move to weekly visits.
Month 2 onwardThe goal shifts to staying well.Spacing visits out as far as keeps you well.

Why it can be faster

Why does Spravato work faster than antidepressants?

Spravato works faster than most antidepressants because it acts on a different system. Most antidepressants adjust serotonin or norepinephrine and take weeks to show their effect. Spravato acts on glutamate, the brain's main signalling chemical, which is why changes can show up within days. It's also why a history of antidepressants that didn't work isn't a prediction about this one: that history is what qualifies you for it.

That's the trade with Spravato: a visit or two a week in our office, instead of a pill every day and a long wait to find out whether it works.

Signs it's working

How do you know Spravato is working?

You know Spravato is working when the everyday things start to come back: getting out of bed is easier, sleep settles, small tasks stop feeling impossible, and the people around you notice before you do. We track this with you at every visit, so you're not left guessing.

Feeling less isn't working less

Gentler sessions don't mean it's stopped.

After the first few treatments, most people barely notice the dissociation. Many mistake that for the medication not working. How strongly you feel a session has nothing to do with how well it's helping.

If it's slower

What if Spravato hasn't worked after four weeks?

If Spravato hasn't worked after four weeks, we look at why with you before deciding anything. Some people respond later, some do better at a different dose, and for some a different next step makes more sense. You'll never be left on a treatment that isn't helping. What happens if Spravato stops working.

A few things give the treatment its best chance in the first month:

  • Keep both visits each week. The benefit builds across the schedule, so the twice-weekly start matters.
  • If you take a daily antidepressant, keep taking it unless your clinician says otherwise. Changes to your other medicines are planned together, not made on the fly.
  • Tell us what you notice, good or bad, at every visit. Small changes in sleep or energy often come before mood.
Once it's working

How long do Spravato's benefits last?

Spravato's benefits last as long as you stay on the schedule that keeps you well, and staying on it protects the progress. In the SUSTAIN-1 trial, people in stable remission who kept taking Spravato were 51% less likely to relapse than people switched to placebo (Daly et al., JAMA Psychiatry, 2019).

That's why, once you're doing well, we don't stop: we space visits out, usually to weekly and then every other week, as far as keeps you well. How often and for how long.

Timing questions

What people ask about how fast Spravato works.

How long does it take for Spravato to kick in?

Many people notice a change within the first days to weeks. In the approval trial, people on Spravato were doing better than the comparison group from day 2.

How many Spravato treatments before it works?

Most people who respond see it within the first month, which is eight treatments at twice a week. How often and how long treatment lasts.

How long does one Spravato dose last?

The effects you feel during a session wear off within hours. The antidepressant benefit builds across the course of treatment, which is why the schedule matters.

Is Spravato faster than a regular antidepressant?

Usually, yes. Daily antidepressants often take 6 to 8 weeks to show their full effect, though a newer pill such as Auvelity can work sooner. In the approval trial, people on Spravato were doing better than the comparison group from day 2.

Does Spravato work right away for everyone?

No. Some people respond within days, others over several weeks, and some don't respond. We check in at every visit and adjust with you.
