Aetna's prior authorization

How does Aetna's Spravato prior authorization work?

Aetna's Spravato prior authorization asks for proof of treatment-resistant depression: the diagnosis, the antidepressants tried and for how long, and the plan for treatment. Depending on the plan, we submit it by phone, by phone with records, or through an online form with records attached. You don't have to do any of it.

  2. We check your planWe confirm how your plan covers Spravato and what your share is.
  3. Your evaluationUsually within one business day. We capture exactly what Aetna asks for.
  4. We get the approvalWe submit it and follow it through, and appeal if needed.

What does Aetna's Spravato policy require?

Aetna's Clinical Policy Bulletin 0950 (last reviewed February 2026) sets out what its commercial plans need to see for treatment-resistant depression. In plain words:

  • Two antidepressants that didn't work, from two different classes, each taken at the highest dose you could tolerate for at least 8 weeks, within the past 5 years.
  • One more step during this episode: either an add-on medicine for at least 8 weeks, or talk therapy such as CBT.
  • Severe depression, measured on a standard rating scale. We score it at your evaluation.
  • A psychiatrist prescribing or consulting, and an office that gives each dose and monitors you for two hours. Both are how we work.
  • If there's a substance or alcohol use problem, it needs to be in treatment. We talk this through openly.

Once you're approved, Aetna renews in six-month steps when your rating-scale scores show improvement, and we track those scores at your visits so renewals go in ready. If your history doesn't line up neatly with the list, tell us anyway: working out what an approval needs is our job, not yours.

If Aetna says no

What if Aetna denies Spravato?

If Aetna denies Spravato, we don't stop. We always request a peer-to-peer review with Aetna's doctor. If that fails, we always file a second-level appeal, and if that fails, we always take it to independent external review. In our most recent internal review, 34 of 36 denials were overturned.
