Cigna and Spravato

Does Cigna cover Spravato? Yes, with an approval we handle.

Cigna covers Spravato for treatment-resistant depression once a prior authorization is approved, and we take care of that request from start to finish. Most of our patients with Cigna pay up to $40 a treatment, and if Cigna turns a request down, we work it through every level of appeal.

What Cigna asks for

What does Cigna's Spravato policy require?

Cigna's Spravato policy requires evidence of treatment-resistant depression. Its drug coverage policy IP0220 (effective June 2026) asks for:

  • Little or no improvement on two antidepressants from different classes, each at a therapeutic dose for at least 6 weeks in this episode of depression.
  • A psychiatrist or mental health provider prescribing or consulting.
  • A check of the state prescription monitoring program, which the prescriber does.
  • A note on any history of psychosis. It doesn't rule you out; the prescriber weighs it.
  • Age 18 or older.

One thing that's different with Cigna: Cigna has the medicine itself supplied by a REMS-certified pharmacy rather than from our office's own stock, and its prior authorization usually takes a little longer than other insurers'. We know the routine and start the paperwork right after your evaluation, so the extra step costs as little time as possible.

The first approval runs for 6 months. After that, Cigna renews for a year at a time if you're benefiting. Our evaluation is built to capture exactly these details, which is why 95% of our requests are approved the first time.

  • Your cost: up to $40 a treatment for most Cigna patients, after the SpravatoWithMe savings program, for those eligible. We confirm it before you start.

  • NJ Medicaid: $0. Initial denials are common, and we’re incredibly persistent about appealing.
  • Commercial insurance: up to $40 after Janssen’s SpravatoWithMe program, for patients who are eligible. Your own plan sets the details.
  • We check before you commit. Insurance verification starts the moment you reach out.

When Cigna says no

What if Cigna denies Spravato?

When Cigna denies Spravato, the next move is ours, not yours. We ask for a peer-to-peer conversation with Cigna's reviewing doctor every time. If that doesn't change the decision, we file a second-level appeal every time, and if that fails too, we take it to independent external review every time. In our most recent internal review, 34 of 36 denials were overturned.
