Spravato horror stories: what's really behind them.
Most Spravato horror stories describe the same thing: the first hour of a first session, when you can feel floaty, detached or unreal. That feeling is expected, it's temporary, and it happens in a recliner with our team beside you. It settles before you go home, and after the first few treatments most people hardly notice it.

Why do people post Spravato horror stories?
People post Spravato horror stories because the first session can feel strange, and strange is frightening when no one prepared you for it. Dissociation, the floaty, detached feeling, happens to 61% to 84% of people in the trials (Spravato Prescribing Information). It's the most common effect there is, and it's temporary.
| What the stories describe | What's actually happening | How long it lasts |
|---|---|---|
| "I felt like I left my body" | Dissociation: the most common effect, and expected rather than a sign that something went wrong. | Strongest early in the first hour, then easing while you rest. |
| "Everything felt unreal" | The same dissociation. Dimming the lights and closing your eyes usually makes it gentler. | Usually gone by the time you leave. |
| "I felt sick and dizzy" | Nausea and dizziness, common in early sessions. Skipping food for two hours beforehand helps. | Short-lived, usually within the first hour. |
| "My heart was racing" | A temporary rise in blood pressure, which peaks about 40 minutes after the dose (Prescribing Information). | Settles over about four hours. We check it before you go. |
| "I was wiped out afterward" | Sleepiness after the dose. | Mostly in the first sessions. After that, most people get on with their day. You don't drive until after a night's sleep. |
Rates and timings are from the SPRAVATO® (esketamine) Prescribing Information, Janssen Pharmaceuticals. How strongly any one person feels each effect varies.
What else do Spravato horror stories describe?
Beyond the first hour, Spravato horror stories tend to fall into five themes. Each one is real for the person who wrote it, and each one is something our team plans for from the first visit.
- "It made my depression worse." Some people feel tearful or low in the first weeks, while the treatment is still building. Like every antidepressant, Spravato carries a warning to watch for worsening mood or suicidal thoughts, especially early on (Prescribing Information). That's one reason we see you twice a week at the start: a Sherpa and a clinician check in with you at every visit, and you can call us between visits at (908) 450-7002. If you're ever in crisis, call or text 988. Crisis resources.
- "I crashed when I missed a dose." Spravato's benefit builds across the schedule, and a missed week in the first months can let symptoms creep back. With four offices on one phone number, a missed visit can usually be made up within days, and we only space visits out once you're steady. How often treatment happens, and for how long.
- "I was wiped out for the rest of the day." Sleepiness after a dose is common in the first sessions, so plan to take it easy on those days. After the first treatment or two, most of our patients find they don't need much rest at all and go back to their usual day, apart from driving.
- "It cost a fortune." Many of the cost stories come from people paying out of pocket for ketamine, or for Spravato at a clinic that left the insurance to them. Spravato is covered by most insurance, and we run the approval ourselves: 95% of our requests are approved the first time, and most of our patients pay $0 to $40 a treatment. Cost and insurance.
- "It didn't work for me." Not everyone responds, and no one should be told otherwise. What we do is make sure you're never left guessing: we judge with you how it's going at around four weeks, adjust the dose or schedule if that makes sense, and talk through other options if it doesn't. What if Spravato stops working.
Does Spravato ruin your life?
No. Spravato's side effects are temporary: they live inside the two hours you spend resting in our office, and most people go home feeling like themselves. That's the opposite of what many people have already lived through with daily antidepressants, whose side effects, such as weight gain, sexual side effects or feeling emotionally flat, can come home with you every single day. It's also different from electroconvulsive therapy (ECT), where memory loss is a known risk and for some people part of it is lasting (FDA, ECT device classification, 2018).

The antidepressants that didn't work aren't a prediction. They're the qualification.

Spravato works on the brain's glutamate signalling, a different system from the serotonin and norepinephrine that most antidepressants act on. That's why it can help people whose history suggests nothing will. One strange hour, twice a week to start, is the trade for a treatment that works differently from everything you've tried.
What happens if your first Spravato session is hard?
If your first Spravato session is hard, we change what we can before the next one, and it usually gets easier. More than 30,000 treatments have taught our team a great many small things that make the first hour gentler.
- BeforeOne of our Sherpas, the specially trained team members who look after you on every treatment day, walks you through what the first hour may feel like, so nothing catches you off guard.
- DuringYou rest in a reclining chair. Eye masks and noise-cancelling headphones are on hand, and music without lyrics helps many people ride the floaty feeling instead of fighting it.
- If it's a lotYour Sherpa stays with you. Company if you want it, quiet if you don't.
- Next timeYour clinician talks through the dose, the timing and what helped, and adjusts the plan with you.
Most people hardly notice it after the first few treatments.
The dissociation in a first session is usually the strongest it will ever be. Many people mistake the later, gentler sessions for the medication not working. Feeling less doesn't mean it's helping less. What Spravato feels like.

Is the dissociation from Spravato dangerous?
Dissociation from Spravato is expected and temporary, not a sign of harm. It's the reason every dose is given in a certified office and you stay with us for at least two hours, with your blood pressure checked along the way. Why Spravato is only given in certified offices.
Will every Spravato session feel like the first one?
No. For most people the first sessions are the most intense, and after the first few treatments they hardly notice the dissociation at all.
Can I stop Spravato if I don't like how it feels?
Yes. You decide whether to continue at every visit, and your clinician can adjust the dose and plan with you first.
Is a Spravato session like a ketamine trip?
No. Spravato is esketamine, given at a measured dose in a nasal spray and monitored for two hours. The feeling is usually milder and shorter than people expect. Spravato vs ketamine.
Why are online Spravato reviews so mixed?
Online reviews are written most often right after a first session, when the dissociation is at its strongest and many people weren't told what to expect. Preparation changes that: knowing what the first hour may feel like, and having someone beside you while it happens, is the biggest difference we see.
Can Spravato cause lasting damage?
In long-term follow-up of people treated for up to about six and a half years, no new safety concerns appeared and there was no evidence of harm to memory or thinking. What long-term data show.
Are there good Spravato stories too?
Many. Across more than 200 Google reviews of our four offices, nearly all are five stars, and patients mention kindness more than anything else. Spravato reviews and patient stories.
